Price: Early Bird Price: $125 per person. After August 1st: $140.00 Fee includes golf, cart, dinner, and drink (please advise of any dietary restrictions) Format: 1 p.m. shotgun, 4-person scramble (best ball), putting contest during the round. Prizes: Longest drive (men and women), closest to the hole (men and women), lowest team score (overall and women only), hole-in-one on the 7th (par 3) and draw prizes. Dress Code: Standard golf attire (i.e. collars on shirts and no jeans). Casual dress for dinner. Deadline: Registration forms (with payment) must be received by August 4, 2016. Entry forms will be processed as they are received so register early! Payment: You can now register & pay online using our online registration – payment is by credit card via PayPal. You can also register by downloading a Printable Registration Form and mailing it with a cheque payable to Breast Cancer Action to the address below OR call 613 736 5921 during business hours with your credit card information and we will be happy to process your registration. Each member of your team must register individually to receive a tax receipt. Thank you!
